Provide care to the elders living in the Crest home and care for their environment by doing laundry, cooking, and housekeeping. Protect, sustain and nurture the elders by providing assistance with activities of daily living (A.D.L.s) and meeting other needs as required. Provide assistance in such a manner as to promote a high quality of life. Coordinate all aspects of the self-managed work team.

* Assist with the appropriate implementation of the prescribed service plan. Supervise and/or administer treatments prescribed by physician.

* Provide for the Elders' personal hygiene including, but not limited to:
cleaning bedpans and urinals, changing briefs, administering appropriate peri-care, administering baths/showers, backrubs, shampoos and shaves.

* Assist with activities of daily living including, but not limited to dressing an elder, cooking meals, assisting elders during mealtime to ensure adequate food intake, turning, and positions Elders; providing fresh water, snacks and nourishment between meals in a timely and accurate manner.

* Appropriately assist with showering and/or baths as scheduled.

* Provide for the comfort needs of Elders in a timely, compassionate and accurate manner.

* Work effectively as a team member in the delivery of care to an Elder.

* Utilize resources and materials in an efficient and safe manner.

* Observe and immediately report elder reactions and changes in condition.

* Maintain a safe and attractive environment for Elders and co-workers.

* Promptly answer Elders' call lights and requests.

* Immediately detect, correct or report situations that have a possibility of causing accidents or injuries to elders or others.

* Through appropriate communication, will maintain excellent working relations among other C.N.A.s, therapists and consultants.

* Be willing and capable to perform, with specialized training and written approval, additional and specialized tasks pertinent to assigned area.

* Attend additional in-service to meet the requirement set for us by Texas Department of Aging and Disability Services.

* Serve as liaison between Elders, families and outside caregivers in order to best minister to the Elders.

* Be willing to become knowledgeable and adhere to the regulations set before us by the State of Texas.

* Be willing to accept and full-fill various coordinators' roles and the responsibilities associated with each role.

* Agree to follow the "no-lift policy and use the lift equipment as instructed.

* Be willing and capable of utilizing the elder documentation system for elder documentation.

* Attend house meetings and abide by house rules.

* Fully knowledgeable of the fire evacuation and disaster plan of the community and participates as assigned.

* Clean and service all aspects of rooms, baths, offices, and common areas seeking a "like new" effect.

* Check and fills dispensers, hand soap, paper

* Make minor repairs and adjustments; replaces light bulbs; plunges clogged sinks, commodes, or drains.

* Set, bus and clean tables after meals.

* Be responsible for preparing food and beverages according elder request.

* Serve food and beverages at meal times and snacks.

* Be responsible for washing dishes; sorts, stacks, stores clean dishes.

* Grocery shop for the house.

* Provide meaningful activities for residents and support other team members in their effort to provide activities.

* Maintain compliance with all BMM and Buckner policies, procedures and requirements. Maintain compliance with all state and federal laws and regulatory requirements.

* Work successfully as part of a team and responsible for developing then maintaining strong working relationships with co-workers.

* Attend and participate in staff meetings and training as required.

* Perform general office tasks as required.

* Perform special assignments, projects, and other duties as required.


NOTE: These requirements represent minimum levels in order to perform the job on a satisfactory basis. Candidates must have the ability to satisfactorily perform the essential functions of the job.

* Requires basic understanding of specialized area of knowledge normally acquired through the attainment of a high school diploma (or G.E.D.) and the successful completion of a Certified Nursing Aide course. Requires ability to understand and carry out moderately complex oral and written instructions.

* Must hold and maintain a current Texas licensure as a Certified Nurse Aide or be willing to obtain certification. Must complete and/or meet training certification required by Buckner and the state of Texas.

* Requires proficient ability to speak, read and write English.

* Must attend additional training of approximately 120 hours. This training at minimum consist of teamwork, empowerment, conflict resolution, customer obsession, CPR, first aide, infection control, culinary, Certified Dietary Mangers course, memory care training.

* Work deals with areas such as Elder records, medical records, visual inspection involving small details. Visual work is primarily close to observer. Depth perception and field of vision (peripheral) are not as critical as the ability to distinguish small details and markings very near to the observer.

* Requires proficient ability to recognize differences in sound, such as voices/noises that are loud and playful instead of angry and combative.

* Requires proficient ability to exercise independent judgment under broad certified nursing care guidelines.

* Requires ability to establish and maintain effective working relationships with other employees and the public. Requires ability to meet emotional and physical demands of Elders (and to help ensure their dignity) and other team players in the house.

* Requires ability to speak clearly and make self understood effectively in face to face interactions; articulate with accuracy to speak on the phone.

* Requires ability to hear and receive verbal instructions, answer phones, communicate with people in situations with some background noise.

* Requires ability to maintain confidentiality.

* Proficient working knowledge and ability to accurately and timely operate and perform computer related tasks with specific equipment and software applications required.

* Requires ability to drive assigned vehicle(s) or personal vehicle, with appropriate state license, following all laws applicable; must provide proof of liability insurance and must be eligible to be insured under BMM/Buckner's insurance policy. Must be age 21 or older to drive on behalf of the Company.

* Requires ability to use up to 50 pounds of force occasionally, and/or up to 20 pounds of force frequently, and/or up to 10 pounds of force continuously to move objects and/or people.

* Requires ability to walk and stand sometimes for prolonged periods of time. Position requires bending, stooping, twisting, turning, reaching, lifting, carrying, pulling, pushing, climbing, kneeling, walking and standing over 75% of shift. Sitting on the job is greatly discouraged.
